Actually it started out as a comic by Mirage Studios, then the TV show, then the game. And the box art is from a comic book TMNT#4 to be specific and the cover featured the turtles with the same color of mask I found it on wikipedia.

This game was based on the comic book version not the television show like the sequels after. That explains why the box art cover had all turtles wearing the same color mask. In the comic book version, the art cover was also close to the same as the video game box art cover but the comic book series itself was only published in black and white at the time. The Teenage Mutant Ninja Turtles were not given tradition color appearance until they make their television debut.
